Level of Care is based on an individual’s need for help with Activities of Daily Living (ADL) and is determined by an assessment done prior to admission.

We know that a person’s needs change periodically and that care required may increase over time. Sunset Communities’ knowledgeable and caring staff is prepared to meet these changing needs while allowing each person to maintain his or her individuality and dignity, and assuring that transitioning between levels is smooth and stress-free.

Assisted Living

The comfort of knowing someone is there “just in case.” Sunset Communities offers multiple levels of service to meet your unique situation and will create a wellness plan that is specific to you and your needs.

Memory Support

At Sunset House and Sunset Village, we are especially proud of our areas dedicated to individuals facing early stage memory loss and symptoms of dementia. These unique neighborhoods are open and homelike, yet keypad protected for safety.

Our well-trained associates provide an innovative and personalized approach to caring for those who need total care in a majority of daily living tasks. Our specialized healthcare centers give first priority to our current residents. So you can find comfort in knowing that skilled nursing services will be available when needed.

Skilled Rehabilitation

Sometimes a temporary medical situation requires inpatient skilled rehabilitation treatment to get you back to your best self. Our therapy program provides comprehensive rehabilitation services, including physical, occupational, and speech therapies in our comfortable and cozy healthcare neighborhoods. Services are available seven days a week, including weekends and holidays, so you can be sure your therapy goals will progress at your pace, not at the convenience of others.

Outpatient Rehab

We offer physical, occupational, and speech therapies in our fully accessible buildings. Our friendly and experienced therapy team is ready to work within your schedule to get you back in action. Ask our admissions staff for more information.

Caring for a loved one with cognitive or physical limitations can take a physical and emotional toll on the caregiver. Let Sunset Communities give you a much-deserved break to rest and recharge. Our admissions staff will be happy to put together a respite plan that meets the specific needs of your loved one and gives you the peace of mind and freedom to take a much needed break. Respite stays can be as short as a weekend or as long as 60 days, and are available at both Sunset House and Sunset Village. Respite accommodations are based on availability.
